Driver's
License:
You will need your current valid State Driver's License, proof of Wisconsin residency, Social Security Number, and proof of signature. You will have to take an eye exam, but a written or driving test is not required. No appointments are needed and fees for
the basic license are $18.00.
If you have lost
your Driver's License:
You are required to show two identifications to prove who you are with your
signature. The fee for duplicate license is $4.00.

According to the new law of September 1, 1998, automobiles, trucks, dual
